Over here in the UK, it's quite commonly used to mean *porn* - the porn magazines are always kept on the top shelf, so they're away from the kiddies, so the phrase tends to be associated with porn more than quality. So I found her work somewhat disappointing ;) “Top Shelf” – means high quality over there.
